We submit the applications to your preferred aged care facilities and work closely with these facilities until we have secured a placement for your loved one.

Our service includes a follow up service after placement to ensure your loved one has settled in well & you are happy. We will communicate with family as well as clinical staff within the facility & help to resolve any teething problems if required.
CORONA VIRUS UPDATE:
In this rapidly evolving health crisis, Aged Care Assistance has implemented an addition to our aged care placement service.
To assist in protecting the vulnerable members of the community, we would like to offer our ‘remote’ service. This service is provided without a face to face meeting. We will gather the required information over the phone & via email or post, to ensure the continuity of our valuable service offering, without any infection control risks. We are also happy to arrange a meeting over the Zoom platform involving all family members, providing the opportunity to ensure consistent information delivery.
We are in daily contact with service providers to receive updates on the infection control policies & procedures, pertaining each provider.
The inclusions of this service are the same as our usual service, other than a face to face meeting.

Quite often, the applicant is a patient within the hospital system. There is a certain expectation from the hospital that you are being pro-active in seeking appropriate care for your loved one. You will often be given a time frame in which the patient is to be discharged. The reason for this is that the hospitals are required to discharge patients who no longer require 'acute care'. Many people become distressed when faced with this situation. We can assist you in urgently placing the applicant into a nursing home bed, either for permanent care or respite care pending a more suitable permanent vacancy which should reduce your stress level, especially if you are a full time employee & have limited time available.
